| Participants are responsible for obtaining any visas (including, if appropriate, for transit) which may be necessary. |
| However, if there is no Senegalese diplomatic or consular representation in your country, to facilitate issuance of a visa we will forward your information to the Senegalese authorities in advance of your departure. |
| Participants in countries where there is no Senegalese embassy or consulate need to do the following to request a visa: |

Please complete the Application for Visa form and return it by fax (to +41.22.909.5931) to the GAVI Secretariat, attn: Partners' Meeting team. |

Fax us photocopies of pages of your passport which indicate:
- name
- date of birth
- passport number
- date of issuance
- validity
- name of authority that issued the passport

| 3. |
Fax us a photocopy of your vaccination certificate showing a valid yellow fever vaccination entry (yellow fever vaccination is required for issuance of a visa). |
| 4. |
On arrival at Dakar Airport you will be required to present:
- your passport
- three passport photographs
- the original + three photocopies of the completed visa form
